The state of Texas requires therapists to keep records of their
clients. Please be as thorough as you can when filling these forms
out. The therapist will also make notes on these forms in order to
make all of your treatments effective and enjoyable.
Open or Download the appropriate form and print it, fill it out,
and bring it with you to your first massage.
